Admission

  1. Individual must be over the age 18 years of age.

  2. Individual must be eligible for Medicaid, meet the Virginia Individual Developmental Disability Eligibility Survey (VIDES) criteria, and have a diagnosis of Developmental Disability (DD).

  3. One of the following Developmental Disability Waivers: Building Independence (BI),Family & Individual Supports (FIS), or Community Living (CL).

  4. The individual must have significant limitations in adaptive skills in two or more of the following areas: communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure and work.

  5. Individual is not known to have tuberculosis in a communicable form.

  6. Individual does not have a history (within 2 years) of creating unsafe conditions such as fire abuse, sexual offenses, or history of abuse of illegal drug substance.

  7. Individual’s active Behavior Management Plan does not require the use of procedures which would conflict with SHCS Residential Program Human Rights Plan: time out, seclusion, intrusive aversive therapy, physical restraints, or mechanical restraints or a need for restrictive behavioral interventions.

  8. The individual’s service needs are available by SHCS Residential Program

  9. Individual’s, who do not meet the characteristics described in 1 through 8, may not be admitted to the program.

  10. If an individual’s status changes and he/she no longer meets the characteristics described in 1 through 8, the individual may be discharged from the program.

Exclusion Criteria:

1.Individual has tuberculosis in a communicable form.

2.Individual requires intrusive behavioral therapies, seclusion, physical restraints, or mechanical restraints.

3.Individual has a history (within 2 years) of creating unsafe conditions such as fire abuse, sexual offenses, or history of abuse of illegal drug substances.

4.Individual’s needs are not available by SHCS Residential Program.

5.Individual does not have a primary diagnosis of intellectual disability/mental illness.

6.If an individual status changes and he/she no longer need nor meet the criteria described in the above statements, the individual may be discharged from the program.
